Who this form involves

Former tenants who moved out after receiving a termination notice and later saw signs the stated reason may not have been genuine.

This is an application a tenant can file with the LTB.

When it may apply

You moved out after an N12, but the unit appears to have been re-rented to someone else instead of used as stated.
You moved out after an N13, but the claimed demolition, major repair, or conversion does not appear to have happened.
The unit was advertised for rent at a higher price shortly after you left.
