This is a notice or agreement a tenant gives or signs — not an application to the LTB.

When it may apply

You want to move out at the end of your lease term.
You are on a month-to-month tenancy and want to leave.
You are ending a tenancy in one of the special situations the law allows (a legal professional can confirm whether one applies to you).
